Core Connection Physical Therapy chooses not to be contracted with any insurance companies, with the exception of Medicare.

That being said, we are still considered an out of network provider and you may be able to receive reimbursement for services from your insurance company. We require all services to be paid up front at the time of service, however we can provide you with an invoice that you would submit to your insurance company.
